How Depressed are you?
The Hospital Anxiety and Depression questionnaire (The HAD Scale), below is one of the most widely used self-rating scales for depression and anxiety. By answering the questions you can get some idea of how depressed or anxious you are.
In using this questionnaire, it is important to remember that it is a measure specifically for anxiety and depression and is not designed to assess any other dimension of how you might be feeling.
Click the number next to the answer that reflects how you have been feeling during the last week. If you cannot decide between two answers, circle the higher number. Don't take too long over your replies; your immediate reaction to each item will probably be more accurate.
